No. Membership is not required to receive care at Evigilant Care. We offer insurance-based and self-pay appointments, as well as optional membership plans for patients who prefer to pay out of pocket or want an enhanced level of access, communication, care coordination, and ongoing support.

Membership benefits vary by plan and may include scheduled medication-management visits, secure messaging for nonurgent questions, medication reconciliation, refill assistance, health-marker monitoring, priority scheduling, and coordination with pharmacies, specialists, therapists, and caregivers.
Laboratory testing, medications, imaging, toxicology screening, and services provided outside Evigilant Care may involve additional costs. Please review the individual membership plans or contact our office for help choosing the option that best fits your needs.

Yes. Self-pay appointments and tiered membership options are available for patients who do not have insurance, choose not to use insurance, or prefer to pay out of pocket. Please contact our office for current pricing and payment information.

Yes. Secure telehealth appointments are available to eligible patients located in North Carolina at the time of their visit. Some services, evaluations, or monitoring requirements may require an in-person appointment. Our team will let you know whether telehealth is appropriate for your specific needs.

Your first appointment will include a comprehensive review of your medical history, current medications, symptoms, treatment experiences, and health goals. Please bring or upload an accurate list of all prescription medications, over-the-counter products, vitamins, and supplements you currently take.
Dr. Obiaja will discuss your concerns, answer your questions, and explain any recommended treatment, monitoring, or follow-up. Additional records, laboratory testing, or coordination with your other healthcare providers may be needed before certain medications can be prescribed or changed.
Controlled medications may be prescribed when clinically appropriate and supported by a complete medical evaluation. Evigilant Care provides buprenorphine for opioid use disorder, benzodiazepines as part of a structured taper plan, and certain other controlled medications for approved indications when appropriate.
Prescribing is never guaranteed. Patients may be required to attend regular follow-up appointments, complete laboratory or toxicology testing, participate in prescription-monitoring review, and follow the practice’s controlled-medication agreement. Evigilant Care does not prescribe opioids for chronic non-cancer pain.
Current patients should submit refill requests through the secure patient portal. Please request routine refills at least three business days before your medication runs out.
Routine requests are generally reviewed within 1–2 business days. Controlled-medication requests may require up to 72 hours and additional clinical review. Processing time does not guarantee approval, and an appointment, laboratory testing, or other monitoring may be required. Controlled medications are not routinely refilled early or after hours.
Evigilant Care focuses on medication management, chronic-condition support, recovery treatment, outpatient benzodiazepine tapering, and medical weight management. These services are not intended to replace comprehensive primary care.
We encourage patients to maintain a relationship with a primary care provider for preventive care, vaccinations, routine screenings, acute illness, and other general medical needs. When appropriate and with your authorization, we can coordinate your medication plan with your primary care provider.

Yes. With your written authorization, Evigilant Care can communicate with your primary care provider, therapist, psychiatrist, pharmacy, specialist, or other members of your treatment team. Care coordination can help reduce medication duplication, prevent interactions, clarify treatment responsibilities, and support continuity of care.

Healthcare professionals may submit a referral through the secure referral form on our website. The referring provider should include their contact information, the patient’s contact information, the requested service, the reason for referral, and any relevant clinical details.
The referring provider must obtain the patient’s permission before sharing protected health information. Additional records may be requested after the referral is reviewed. Submission of a referral does not guarantee acceptance into treatment. For urgent clinical concerns, please contact our office directly at 919-364-5203.
